The Solomon Assessment Center at Boys & Girls Homes is an emergency placement shelter for children ages 10 - 17. With 24/7 admissions, placements are limited to 90 days, during which individualized treatment plans will be developed for the youth and updated bi-weekly.
Youth will be connected to medical, dental, mental health and educational services while in care.
The daily schedule will typically include recreation activities of daily living, education opportunities and group process activities.
Upon discharge, the guardian will receive a summary that includes:
- Date and discharge outcome
- Evaluation of services provided
- Recommendation for services needed
- Provision/referral for aftercare
- Copies of all medical, dental, educational, clinical records
We begin discharge planning at placement. This program is meant to be a transition into getting the children back home, into placement or recommend them to a higher level of care.
